This is the self form for a students that can help to change combination for A level students (form five and six). Also to change college courses.
How to use and change Combination for form five (advanced) students ?
the following are the instructions that will guide students on changing subjects:

Go to selform.tamisemi.go.tz

Login to your account by filling username and password if you have already registered.

If you are not a member register a new account here selform.tamisemi.go.tz/CandidateRegistration/

Fill your form four index number. Example: S.0001.0321.2020

Click verify and it will redirect you to another page that will ask for filling your information’s.

Select your grade subjects (Kiswahili, physics, chemistry, biology, basic mathematics, English language, geography, history etc.)

Fill your CORRECT full names and date of birth

Click register
-
If the information’s filled are correct click Register and the new page will be open
-
Create new passwords that it is simple to remember and save.
